What's New in Nail Management: Malignant Onychopapilloma
Description
In this presentation, the speaker discusses malignant onychopapilloma, a newly identified condition in nail pathology. Despite being a recent topic with only a few papers published, it is crucial for dermatopathologists to recognize its clinical and histopathological features, which can mimic benign onychopapillomas. The initial study by researchers from Bern highlighted atypical cells found in the keratogenous zone and differences in histological findings compared to Bowen's disease and onychocytic carcinoma. New terminology such as "atypical onychopapilloma" is introduced, reflecting caution in labeling as malignant due to differing clinical presentations like longitudinal melaninicia and distal subungual hyperkeratosis. The importance of specific markers like P53 and Ki-67 for evaluation is emphasized, along with reassurance that most onychopapillomas remain benign. The speaker invites the audience to an upcoming workshop in Amsterdam and mentions ongoing educational efforts in nail pathology. Overall, the session aims to enhance awareness of this emerging condition within the medical community.

- Malignant onychopapillomas are a newly recognized entity with distinct histopathological features.
- They present clinically similar to traditional onychopapillomas, making diagnosis challenging.
- A small percentage of onychopapillomas (approximately 3.3%) may exhibit atypical features suggestive of malignancy.
- Histopathological examination can reveal increased expression of p53 and Ki67 in atypical cases.
- Malignant onychopapilloma is not associated with high-risk HPV infection, differentiating it from other nail diseases.
- The introduction of the term 'atypical onychopapilloma' reflects uncertainty regarding the malignancy status of some cases.
- Differential diagnosis includes distinguishing features related to onychocytic carcinoma and ungual Bowen disease.
