Update: Diagnosis and Management of Nail Psoriasis
Description
The presentation provided an update on the diagnosis and management of nail psoriasis. Diagnosis is generally straightforward when associated with skin disease but can be challenging in cases of isolated nail involvement. New diagnostic techniques include nail clipping, which can differentiate nail psoriasis from onychomycosis by identifying neutrophils, and simple X-rays to detect associated psoriatic arthritis. Onychoscopy was discussed, including findings like pseudo pits that may emerge as a significant indicator of nail psoriasis. Various treatment approaches were highlighted, emphasizing individualized care based on disease characteristics and patient factors. For mild cases, topical agents like tofacitinib ointment and cyclosporine preparations showed promising results. Methotrexate and lasers were also discussed as effective treatments. Systemic therapies, particularly interleukin inhibitors such as Brodalumab, Ixekizumab, and Bemakizumab, displayed impressive efficacy in treating nail psoriasis. Emerging therapies were noted as potential advancements in managing this condition. Overall, the presentation emphasized the importance of tailored approaches and the evolving landscape of nail psoriasis treatment.
View moreConclusions
- Nail psoriasis is prevalent in a significant number of skin psoriasis patients, yet its isolated presentation remains uncommon.
- Diagnosing nail psoriasis can be particularly challenging in cases without associated skin lesions.
- New diagnostic tools, such as nail clippings, may help differentiate nail psoriasis from other conditions like onychomycosis.
- The presence of neutrophils is a key histological indicator for nail psoriasis.
- Pseudo-pitting may serve as a pathognomonic feature for nail psoriasis.
- Treatment for nail psoriasis should be tailored to individual patient needs based on various factors including severity and patient preferences.
- Topical treatments like tofacitinib and cyclosporine show promise in preliminary studies for managing resistant nail psoriasis.
- Methotrexate has shown a favorable safety profile and efficacy compared to traditional steroids in treatment.
- Lasers combined with topical treatments yield promising results in nail psoriasis management.
- Biologics like Brodalumab, Secukinumab, and Bimekizumab have demonstrated high efficacy and rapid onset in treating nail psoriasis and are effective in improving patient quality of life.
- Network meta-analyses suggest that certain biologic treatments may outperform others in achieving complete nail psoriasis resolution.
- Emerging therapies are anticipated to enhance treatment options for nail psoriasis in the future.
