Retrospective Studies on RF Microneedling Complications and CO2 Laser Treatment of Rhinophyma with Isotretinoin
Description
The speaker presented two retrospective studies. First, they reviewed FDA MAUDE reports on RF microneedling complications from 2020 to 2025 and found 86 device reports, with most events concentrated in one heavily used device. The most common problems were fat loss, scarring, post-inflammatory erythema, and nerve-related injury, which may relate to device design, depth inaccuracies, lack of impedance feedback, operator technique, and overly aggressive treatment settings. The speaker emphasized careful device selection, training, informed consent, and real-time impedance monitoring to reduce risk. Second, they described an eight-year retrospective study of 15 rhinophyma patients treated with fully ablative CO2 laser while also receiving perioperative isotretinoin. The protocol showed good outcomes, with no delayed healing, no significant scarring or dyspigmentation, only minor adverse effects such as milia and one case of mild hypergranulation, and high patient satisfaction. Blinded reviewers found improvement in severity scores, and many patients reported better breathing and strong willingness to recommend the treatment. The overall conclusion was that fully ablative CO2 laser with concurrent low-dose isotretinoin appears safe and effective for rhinophyma and may help challenge traditional concerns about combining isotretinoin with procedural treatments.

- The MAUDE database analysis suggests that RF microneedling can cause serious adverse events such as fat loss, scarring, burns, erythema, and nerve injury, so it should be treated as a true medical procedure requiring trained operators and careful device selection.
- Devices with real-time impedance feedback may help reduce RF microneedling complications by improving control over depth and energy delivery.
- The apparent concentration of adverse-event reports in certain RF microneedling devices may reflect market penetration, lack of impedance control, and operator inexperience rather than device class alone.
- Fully ablative CO2 laser resurfacing combined with perioperative low-dose isotretinoin appears to be safe in rhinophyma patients based on this retrospective series.
- In the rhinophyma cohort, complications were minimal and no delayed healing, significant scarring, infection, or dyspigmentation was observed.
- Patients treated with fully ablative CO2 plus isotretinoin reported very high satisfaction, improved quality of life, and frequent breathing improvement.
- Blinded reviewer scores and follow-up images showed clear reductions in rhinophyma severity after treatment.
- The study suggests low-dose isotretinoin can be continued around fully ablative CO2 treatment without the classic concern for excessive wound-healing complications.
- Overall, the presentation argues that RF microneedling requires more caution and better technology, while fully ablative CO2 laser with low-dose isotretinoin is a promising option for rhinophyma.
- Further prospective, controlled studies are needed to confirm the safety and effectiveness of CO2 laser resurfacing with isotretinoin and to better define RF microneedling risk factors.
