New Advances in Rosacea and Seborrheic Dermatitis

Description

The presentation covers recent advancements in understanding and treating seborrheic dermatitis (Seb Derm) and rosacea, emphasizing the need for improved treatment approaches due to their commonality and impact on quality of life. Seb Derm, characterized by inflammation in areas with numerous sebaceous glands, is often underdiagnosed, with a prevalence that could reach 10%. Major treatments currently rely on topical antifungals and steroids, which can exacerbate rosacea. The recent FDA approval of rhoflumilast 0.3% foam offers a promising new option, showing significant efficacy with low adverse effects. Meanwhile, ongoing research aims to clarify the disease's pathophysiology, spotlighting the role of immune responses and skin barrier dysfunction. Rosacea presents as a chronic inflammatory condition with varied clinical phenotypes, also suffering from underdiagnosis, particularly in skin of color. While some treatments exist, they often target microbial dysregulation without addressing underlying causes. Advances in treatment may come from understanding the complex inflammatory mechanisms driving rosacea, including TH-1 and TH-17 cell involvement and neurogenic inflammation. Future research must focus on identifying disease biomarkers and refining therapeutic strategies, recognizing the systemic implications of these skin conditions and striving to enhance patient management.

Conclusions

  • Seborrheic dermatitis is one of the most common inflammatory skin diseases, particularly prevalent in patients of color.
  • New insights into the pathophysiology of seborrheic dermatitis include Th17 polarization with Th1 and Th22 upregulation and a notable lack of Th2 dysregulation.
  • Seborrheic dermatitis presents with barrier dysfunction that differs from that observed in atopic dermatitis.
  • The role of Malassezia in seborrheic dermatitis is still unclear and requires further study.
  • Roflumilast 0.3% foam has been recently approved as a safe and effective treatment for seborrheic dermatitis.
  • Investigator-initiated studies are exploring new treatment options such as topical ruxolitinib, PDE4 inhibitors, and IL-12/23 targeting therapies.
  • Rosacea is characterized by increased Th1 and Th17 cell activity and presents with varied clinical features affecting quality of life.
  • There is a need for personalized treatment approaches for rosacea due to its complexity and varied endotypes.
  • Both rosacea and seborrheic dermatitis share an association, impacting potential treatment strategies for affected patients.
