Intralesional Treatments in Androgenetic Alopecia

Description

Dr. Sergiovano Galvan discussed various intralesional treatments for androgenetic alopecia, emphasizing that mesotherapy is generally less effective than oral treatments and is not regarded as a first-line treatment. However, it is favored by patients for its perceived safety and lack of systemic side effects. Galvan highlighted the application of mesotherapy with anti-androgens like dutasteride and bicalutamide, noting that while treatment shows improvement in most patients, its efficacy is lower compared to oral alternatives. Procedures are performed with minimal discomfort through anesthetic blockage, and sessions typically occur two to three times a year. He also touched on the use of PRP (Platelet-Rich Plasma) in conjunction with these therapies, noting a preference for mesotherapy over PRP. Meanwhile, while botulinum toxin has been used to improve scalp conditions and elicit various physiological benefits, its impact on hair density is limited. Galvan mentioned that other agents such as monoxidil and melatonin have not demonstrated significant results. He acknowledged the potential future use of exosomes in mesotherapy, pending further safety studies. Lastly, Galvan invited attendees to future congresses including the World Congress for Hair Research in Madrid in 2028, expressing optimism for upcoming events.

Conclusions

  • Mesotherapy is generally less effective than oral treatments for androgenetic alopecia.
  • Mesotherapy is perceived as safe with low systemic side effects, making it appealing to patients wary of oral medications.
  • Mesotherapy can be utilized in patients who prefer alternatives to oral antiandrogens or require more intensive treatment.
  • In clinical practice, dutasteride mesotherapy has shown an 86% improvement rate with marked improvement in 38% of cases, while side effects were mostly localized pain without serious systemic effects.
  • Bicalutamide mesotherapy showed slight hair density improvement and scoring satisfaction among patients, particularly for seborrhea, but requires further investigation for efficacy.
  • PRP treatment efficacy is still unclear, being potentially more effective in combination with other treatments.
  • Botulinum toxin's effectiveness for hair regrowth was proven inconclusive, suggesting more studies are needed regarding its dosage and application frequency for androgenetic alopecia.
  • Other agents such as minoxidil and melatonin have limited efficacy or painful application, suggesting the need for additional research into exosome treatments.
