Innovations in Dermatologic Laser and Cosmetic Treatments
Description
The speaker reviewed a range of newer dermatologic and cosmetic treatment approaches, emphasizing both expected and serendipitous benefits discovered through clinical use. A picosecond 755 nm laser with a FOCUS array unexpectedly improved photoaging, skin texture, and wrinkling while also stimulating collagen and elastin, leading to a small clinical trial. He also discussed combining low-level 1927 nm fractional treatment with aminolevulinic acid and red light as a well-tolerated option for actinic damage, offering good cosmetic results with only a few days of downtime. Another featured technology was thermomechanical treatment using a heated titanium plate for periocular puffiness and dry eye, described as safe, quick, and maintenance-based. He noted evidence that repeated non-ablative fractional laser resurfacing may help prevent non-melanoma skin cancer, especially in patients with prior basal cell carcinoma and significant sun exposure. For perioral rhytides, he described using RHA fillers to improve upper lip lines while preserving a natural appearance. He also highlighted low-level light photobiomodulation, now FDA approved, with histologic evidence of reduced inflammatory markers such as IL-6. Additional topics included oral tranexamic acid for melasma, often combined with pico laser treatments, a sequential no-downtime regimen using Botox, hyaluronic acid, and pico laser, and newer Erbium:YAG “super smooth” technology that provides rapid resurfacing with minimal downtime. He ended with a humorous anecdote about a monkey accepted into a troop after bonding with a stuffed toy.

- The presentation concludes that picosecond 755 nm laser with a focus array can improve not only pigment but also collagen remodeling and photoaging-related wrinkles.
- It suggests that mild 1927 nm treatment combined with red-light PDT can produce meaningful cosmetic improvement with only minimal downtime, sometimes in a single session.
- The speaker concludes that thermomechanical periorbital treatment can safely reduce under-eye puffiness and improve dry eye symptoms, with maintenance treatments needed every few years.
- Repeated non-ablative fractional laser resurfacing appears to help prevent new non-melanoma skin cancers, including basal cell carcinoma, in high-risk patients.
- RHA fillers are presented as especially useful for perioral and upper-lip rhytids because they preserve more natural lip movement and yield more natural-looking results.
- Photobiomodulation with amber/infrared LED light is described as genuinely effective, with histologic evidence of new collagen formation and reduced inflammatory markers such as IL-6.
- Oral tranexamic acid, especially when combined with pico laser treatment, is concluded to be an effective way to control melasma even though melasma is not cured.
- Sequential combination treatments such as Botox, hyaluronic acid, and pico laser can deliver rapid improvement with little downtime, even in smokers.
- New hybrid Erbium:YAG SuperSMOOTH technology is portrayed as a fast, low-downtime option that may outperform older non-ablative approaches for selected patients.
- Overall, the talk argues that combining devices and modalities often produces better cosmetic outcomes than relying on a single treatment alone.
- After 4 treatments amber/IR LED FDA phase III clinical trial (2004).
