Emerging Topical Therapies: Nonsteroidal Treatments, JAK/PDE4 Inhibitors, CTCL Innovations, and Anti-Aging Approaches
Description
The talk highlighted emerging topical therapies, with a strong emphasis on nonsteroidal treatments as safer alternatives to topical corticosteroids, which can cause local side effects, systemic effects, and steroid withdrawal. It reviewed recent evidence supporting advanced targeted therapies for inflammatory skin disease and atopic dermatitis, including topical calcineurin inhibitors, JAK inhibitors, and PDE4 inhibitors. Ruxolitinib cream was noted as FDA-approved down to age 2 for atopic dermatitis and was also discussed for prurigo nodularis and hidradenitis suppurativa, while delgocitinib showed promise for chronic hand eczema and possible off-label uses such as frontal fibrosing alopecia and CTCL. The speaker also described rising use of topical JAK and PDE4 agents such as ivarmacitinib, roflumilast, and a newly approved PDE4 inhibitor, noting good efficacy, itch reduction, prolonged disease control, and favorable safety without lab monitoring. In CTCL, hypericin gel with photodynamic therapy showed encouraging lesion improvement and potential comparison with mechlorethamine. The final section covered anti-aging and photodamage approaches, focusing on nanodiamond zinc oxide technology, which may improve hyperpigmentation, collagen synthesis, wound healing, skin barrier function, and possibly help prevent actinic keratosis and squamous cell carcinoma by modulating inflammatory and tumor-suppressive pathways.
View moreConclusions
- The presentation concludes that topical nonsteroidal therapies are increasingly preferred because they can treat inflammatory skin disease effectively while avoiding many steroid-related adverse effects.
- It suggests that advanced topical nonsteroidal agents are especially useful for atopic dermatitis, improving itch, quality of life, and longer-term disease control with good tolerability and no routine lab monitoring.
- The talk indicates that topical ruxolitinib is effective and safe for mild-to-moderate atopic dermatitis in children as young as 2 years old.
- It concludes that topical ruxolitinib also shows promising benefit in prurigo nodularis and hidradenitis suppurativa, with meaningful reductions in itch and lesion burden.
- The presentation suggests that topical JAK inhibitors may have broad off-label potential across several difficult dermatoses, although much of the evidence remains limited to case reports or early studies.
- It concludes that delgocitinib is an effective topical option for chronic hand eczema and may also have promise in frontal fibrosing alopecia and other off-label inflammatory conditions.
- It suggests that roflumilast cream is an effective steroid-free therapy for atopic dermatitis in young children and may be expanding into even younger age groups.
- The presentation concludes that hypericin photodynamic therapy may be a useful new topical approach for early-stage cutaneous T-cell lymphoma, with lesion improvement seen over time.
- It suggests that diamond core nanodiamond-zinc oxide technology may help manage photodamage and actinic injury while potentially supporting wound healing and reducing carcinogenic risk.
- The talk concludes that these emerging topical therapies collectively represent a major expansion of the dermatology toolbox and are likely to remain important in future practice.
