Dermatologists Leading Cardiovascular Risk Detection and Statin Management in Psoriasis
Description
The speaker argues that dermatologists should take the lead in detecting and managing cardiovascular risk in patients with psoriasis. He emphasizes that psoriasis is linked to increased cardiovascular events, mortality, and years of life lost, even at younger ages, and that disease severity correlates with higher risk. He notes that cholesterol and other risk factors are often underrecognized and undertreated, while statin use is surprisingly low in high-risk psoriasis patients. Citing recent guidelines, he says psoriasis is a cardiovascular risk enhancer and that dermatologists are authorized to assess risk and prescribe statins, especially since primary care referral has not reliably closed the gap and many primary care doctors are unaware of the association. He also stresses that statins are safe, effective, and underutilized, lowering LDL and reducing heart attack, stroke, and mortality risk. The talk concludes that holistic psoriasis care should include lipid screening and statin management by dermatologists.
View moreConclusions
- Psoriasis appears to be a marker of systemic inflammatory and atherosclerotic disease, not just a skin disorder.
- People with psoriasis have higher cardiovascular morbidity and mortality, including earlier and accelerated risk beginning at younger ages.
- Greater psoriasis severity predicts worse outcomes, including increased death and cardiovascular events in a dose-dependent way.
- Dermatologists can estimate cardiovascular risk by assessing psoriasis extent and severity during routine skin exams.
- Traditional cardiovascular risk factors such as hyperlipidemia are underdiagnosed and undertreated in patients with psoriasis.
- Statins are substantially underused in psoriasis patients who meet cardiovascular-risk criteria.
- Current guidelines and risk-enhancer frameworks support cardiovascular risk assessment and statin consideration in psoriasis patients.
- Relying solely on primary care to manage cardiovascular risk in psoriasis has not been effective.
- Patients and dermatologists are generally receptive to dermatologist-led lipid screening and statin initiation.
- Statins are presented as safe, effective, and capable of meaningfully lowering cardiovascular events, so dermatologists should help lead this preventive care.
