Cutaneous T-Cell Lymphoma: Cases and Early Stage Treatment
Description
This presentation on cutaneous T-cell lymphoma (CTCL) was delivered in the absence of Dr. Malakowski, who shared his slides for discussion. The talk began with a brief overview of CTCL, emphasizing its classification as a group of non-Hodgkin lymphomas primarily affecting the skin, with mycosis fungoides being the most common form. The presenter discussed the importance of diagnosing CTCL using clinical examination and biopsy, noting that classic symptoms include pink-red patches generally located in sun-protected areas. Several clinical cases were shared to illustrate diagnostic challenges and treatment strategies for CTCL. The first case involved a patient misidentified with a benign lesion that ultimately warranted close observation after further evaluation. The second case pertained to a patient with a suspected drug reaction, where the high lymphocyte count required a consideration of multiple diagnoses, ultimately leaning towards a diagnosis of Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS). The third case featured lymphomatoid papulosis, reinforcing the need for thorough pathology review despite lost samples. Treatment options for early-stage CTCL were outlined, stressing the use of topical therapies such as corticosteroids and calcineurin inhibitors, along with newer options like myclorethamine and Hypericin PDT. The session concluded by affirming that a comprehensive clinical approach, combining patient history, pathology, and molecular data, is critical for effective diagnosis and management of CTCL.

- Cutaneous T cell lymphoma (CTCL) is a broad category of skin malignancies primarily of T-cell origin, with mycosis fungoides being the most common subtype.
- Early diagnosis of CTCL is crucial due to its good prognosis, which can alleviate patient concerns about severe outcomes associated with the diagnosis.
- Diagnosis of CTCL is often a clinical-pathological correlation and may require multiple biopsies from different skin areas to confirm.
- Treatment of early-stage CTCL (stages IA and IB) primarily focuses on symptom control and reducing the risk of progression, with a variety of topical agents available.
- Topical therapies such as corticosteroids, calcineurin inhibitors, nitrogen mustard, and retinoids are effective for managing early-stage mycosis fungoides, achieving varying rates of complete remission.
- Novel therapies like hypericin photodynamic therapy (PDT) show promise and are currently under investigation for their effectiveness in treating early-stage CTCL.
- Close monitoring is essential in uncertain cases to rule out malignancies and assess for potential progression in CTCL.
